For your convenience and to save time at your first visit with Water Tower Dental Care, we recommend completing these forms and either submitting them online or printing them at home.
Please remember to bring your dental insurance card and a form of identification to copy for your records. If you have any problems getting these files to submit or print properly, please call our Chicago dental office and we will gladly send or fax them to you.

Full series of digital photos to evaluate the health of your gums and health

Complete exam and decay diagnosis with or without the non-invasive Diagnodent laser

Oral Cancer screening using the Velscope technology

Digital X-rays (90% less radiation compared to conventional x-rays)

Perio/Gum Tissue analysis to determine the type of cleaning needed

One on one discussion in regards to results of your thorough examination and any proposed treatment, complete with written treatment plan
